Client Intake Forms

Client Intake Forms are drafted in Word format.
Download the files to your computer and enter the needed information.

I send the forms with the confirmation email after the client books and pays for the appointment. I ask clients to fill the forms and email them back to me before their appointment.

The forms also can be printed and filled by hand, then scanned and emailed or brought to the office at the time of the appointment, if you see clients in person.
